Allied Health Assistant Gr 2 (IN29) - Social Work Department

About the role

The Alfred Care Group Social Work Service operates across The Alfred, Caulfield and Sandringham Hospitals. The Social Work Service includes approximately 52 EFT (operational reports) with an additional 70 EFT (professional reports) who provide interventions to inpatients and outpatients across all Alfred Care Group clinical programs.

Responsibilities

• Supporting patient discharge planning with paperwork relevant to VPTAS, HARP, ACAS, TCP & MAC and understanding of NDIS support.
• Making referrals to external service providers.
• Sourcing, collating, maintaining and updating ward and department resources.
• Managing enquiries from patients, other hospital staff and visitors.
• Maintaining departmental forms and Social Work resources.
• Maintaining resource files and active management of online resources.

Qualifications

• Relevant health experience or working in a Social welfare setting either in a hospital or community setting.
• Currently studying Social work.

Skills

• Experience in assisting with paperwork relevant to patient discharge planning.
• An understanding of Council resources, meal options, and Centrelink information and accommodation options.
• Ability to maintain departmental forms and Social Work resources.
• Knowledge of managing enquiries from patients, other hospital staff and visitors.
• Understanding of managing online resources.
