Aging Specialist

The Aging Specialist serves as the primary point of contact for older adults, caregivers, professionals, and community members seeking information, assistance, and access to aging services and community resources. This position provides person-centered information and referral services requiring specialized knowledge of aging-related programs, benefits, and services available to older adults and caregivers.
Responsibilities
- Addresses requests for information and referral/assistance with professionalism and expertise; evaluates client needs, identifies appropriate resources and services; makes necessary referrals; conducts follow-up; and utilizes forms and automated systems in compliance with service standards, state reporting requirements, and the Agency's established policies and procedures.
- Manages and sustains confidential client records, ensuring they contain vital information for emergencies.
- Confirms that the records align with the requirements of grants and programs.
- Keeps the essential program documentation up to date.
- Evaluates the requirements of clients; identifies the resources that can be utilized to fulfill those needs.
- Responds to urgent situations, including instances of abuse, neglect, or suicidal thoughts, ensuring that clients obtain prompt support or are referred to the relevant departments or agencies.
Requirements
- Bachelor's degree in Social Work, Human Services, Gerontology, Psychology, Sociology, Public Health, or a related field and two (2) years of professional experience providing information, assistance, support, or services to older adults, caregivers, individuals with disabilities, or other vulnerable populations in a social or human services setting; or an equivalent combination of education and experience.
Preferred Qualifications
- Master's degree in Social Work, Gerontology, Human Services, Public Health, Psychology, Counseling, or a related field.
- Experience providing information and referral services, case management, care coordination, discharge planning, or related human services.
- Knowledge of aging services, community resources, public benefits, and programs available to older adults and caregivers.
- Experience working with individuals and families experiencing complex needs or crises.
- Experience using electronic case management or client tracking systems.
- Experience working in aging services, healthcare, long-term care, or a community-based setting.
- Bilingual or multilingual communication skills are preferred.

Schedule
This position is located at PRCS, 742 Miller Drive SE, Leesburg, VA.
Must undergo and pass pre-employment background checks, including criminal, credit, and DMV checks. A valid driver’s license and a good driving record are required. Annual reviews of driving records may be conducted to ensure ongoing qualification; complete department- and state-required training as necessary.
