After Hours Nurse - Part Time (Weekends)

About the Role

Under the supervision of the Patient Care Manager, the After Hours Nurse provides clinical nursing support during non-core business hours. This includes telephone assessments, home visits, and documentation, all in accordance with hospice standards and procedures.

Responsibilities

  • Uses clinical judgment to assess needs of the patient and their families and initiates intervention to meet immediate needs by utilizing principles of palliative care, including:
    • Assessing the need for skilled nursing by phone
    • Making home visits to calm a situation, provide necessary skilled interventions, and when a patient expires
    • Giving pertinent report to attending physician and/or obtaining new medication orders when indicated
    • Reporting administratively complicated situations to the Patient Care Coordinator
    • Seeking consultation from patient’s primary care physician or Hospice Hawai’i Medical Director and implementing orders/recommendations
    • Making routine/follow-up calls or home visits to new admissions and current patients while on duty, utilizing backup system as needed
  • Obtains updated patient information and physician orders by:
    • Accessing report via the voicemail system or directly from the RN Case Managers
    • Reviewing patient medical records in the EMR system or from another On-Call RN
  • Clears calls with answering service within 10 minutes of receiving a phone message
  • Documents all phone calls, home visits, and medication orders in accordance with Hospice Hawai’i protocol
  • Provides clinical report to Interdisciplinary Team regarding all occurrences during after-hours shift and interventions done for those occurrences by 7:00 AM
  • If patient calls and/or visits are initiated within the last hour of the shift, updates and redistributes call logs to staff before the end of the work shift
  • Assumes the role of either primary or backup call when other After Hours staff is on leave
  • Performs other related duties as assigned by the Patient Care Manager or Director of Clinical Operations

Requirements

  • Valid Registered Nurse License in the State of Hawai’i
  • 3 years of experience as a staff nurse in an acute hospital or comparable, relevant experience
  • Valid Hawai’i Driver’s License
  • Reliable transportation with valid registration and insurance
  • TB Clearance
  • CPR and BLS Certification

  • Ability to exercise independent and sound judgment in planning and providing patient care
  • Ability to problem-solve, demonstrate reason, and analyze situations
  • Skilled in providing holistic nursing care to patients
  • Ability to identify and provide for psychosocial needs of caregivers
  • Ability to be flexible and organized in planning
  • Ability to establish and maintain effective working relationships with staff, clients, family members, the public, and representatives of other agencies and organizations
  • Ability to communicate effectively with all levels of individuals
  • Basic computer knowledge of EMRs and MS applications such as Word, Excel, and Outlook

Working Conditions

  • Indoors in patient homes, nursing, and acute care facilities
  • Outdoors in all types of weather

Schedule

  • Saturday and Sunday, either 8am-4pm, 4pm-12am, or 12am-8am

Pay

$44.71 - $47.60 per hour
