Palm Medical Centers is a well-established provider of value-based primary care services. We are seeking to hire a Primary Care Nurse Practitioner to join our premier team.

Responsibilities

Requirements
- At least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Active and unrestrictive Florida APRN license
- DEA license or eligibility to obtain it within 2 months of date of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
- Proficient with EMR systems (eCW a plus!)
Qualifications
- Bachelor's degree in Nursing
- Current and unrestricted Florida APRN license
- DEA license or eligibility to obtain it within 2 months of date of hire
