Responsibilities
- Show compassion and genuine kindness with patients
- Deliver personalized, preventative care
- Manage chronic diseases and quality metrics in a value-based care model
- Document effectively and build strong patient relationships
- Use EMR systems (e.g., eCW) proficiently
- Participate in value-based insurance contracts and workflows
Requirements
- At least 2-3 years' experience in primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Active and unrestrictive Florida APRN license
- DEA license or eligibility to obtain it within 2 months of date of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
- Proficiency with EMR systems (e.g., eCW preferred)
Qualifications
- Graduate from an accredited nurse practitioner program
- Passing score on the NCLEX-RN
