Rounding on a consistent panel of SNF residents
Manage chronic conditions, med changes, and acute issues on-site—helping facilities avoid unnecessary ER trips when it's appropriate to treat in place.
Thorough visits
- History, exam, care planning, family and staff communication
Collaboration
- Work alongside facility nursing staff
- Partner with psych, palliative care, and neurology teams when needed

Who we're looking for
- Licensed NP or PA in good standing
- Comfortable in a SNF or post-acute setting, or ready to learn
- Background in geriatrics, internal medicine, family practice, home health, hospice, or SNF/LTC work (optional, but preferred)
- Solid clinical judgement and comfortable communicating with facility staff, families, and interdisciplinary teams
- Valid driver's license and a reliable car
Pay Range for Posted Region
$140,000 USD - $200,000 USD
