Rounding on a consistent panel of SNF residents

Manage chronic disease, med changes, and acute issues on-site — helping facilities avoid unnecessary ER trips when it's appropriate to treat in place.

Thorough visits

• History
• Exam
• Care planning
• Family and staff communication

Collaboration

• Work alongside facility nursing staff
• Partner with psych, palliative care, and neurology teams when needed

Qualifications

• Licensed NP or PA in good standing
• Comfortable in a SNF or post-acute setting, or ready to learn
• Background in geriatrics, internal medicine, family practice, home health, hospice, or SNF/LTC work (optional if coachable and willing to build skills)
• Solid clinical judgement and comfortable communicating with facility staff, families, and interdisciplinary teams
• Valid driver's license and a reliable car

Pay Range for Posted Region

$140,000 USD - $200,000 USD
