Rounding on a consistent panel of SNF residents
Manage chronic conditions, med changes, and acute issues on-site.
Help facilities avoid unnecessary ER trips when it's appropriate to treat in place.
Thorough visits
History, exam, care planning, family and staff communication.
Collaboration
Work alongside facility nursing staff, plus our own psych, palliative care, and neurology teams.
Requirements
- Licensed NP or PA in good standing.
- Comfortable in a SNF or post-acute setting, or ready to learn.
- Background in geriatrics, internal medicine, family practice, home health, hospice, or SNF/LTC work is a plus, but not required.
- Solid clinical judgement and comfortable communicating with facility staff, families, and interdisciplinary teams.
- Valid driver's license and a reliable car.

Pay Range for Posted Region
$140,000 USD - $200,000 USD
