Advanced Practice Provider (Per Diem)

About the Role

Under the supervision and in collaboration with a licensed physician who is a member of the active medical staff, the Advanced Practice Provider (APP) performs medical duties for the hospital and participates in patient management as a member of the treating team within the APP's scope of practice. The APP provides general care and treatment to patients, collaborates with physicians and interdisciplinary team members, participates in patient and family education, and contributes to quality improvement, professional development, and hospital initiatives.

Responsibilities

• Patient Care
  • Provide general care and treatment of patients within the scope of practice of an APP
  • Obtain medical histories and perform physical examinations in collaboration with physicians and other members of the treating team
  • Develop diagnostic and treatment plans on admission, including laboratory tests, X-rays, consults, and referrals
  • Prescribe medications in accordance with state and federal regulations
  • Evaluate patient responses to interventions and modify treatment plans as needed
  • Respond to urgent problems in the hospital as appropriate
  • Perform procedures and other tests within prescribed limits and approved clinical privileges
  • Incorporate research data into practice as appropriate
• Interdisciplinary Care
  • Participate in unit team meetings in collaboration with the attending physician
  • Participate in family conferences and treating team conferences
  • Provide patients and families with relevant and understandable information about their illness and ongoing treatment
  • Collaborate with team members to develop and implement comprehensive therapeutic plans
  • Observe patients in therapy departments as needed
  • Collaborate with patients, families, and the care team to develop discharge plans and post-discharge services as needed
• Education and Professional Development
  • Participate in patient and family education activities
  • Provide education and support to staff and students
  • Serve as a clinical resource to staff and students
  • Participate in educational offerings to maintain and expand clinical knowledge
  • Support research-based practice through the development of research studies
  • Participate in professional organizations, committees, meetings, and other educational opportunities as appropriate
• Administrative and Hospital Responsibilities
  • Participate on medical staff and hospital committees as requested
  • Assist with patient unit management as needed
  • Provide clinical consultation
  • Collaborate with the Medical Director in formulating protocols as needed
  • Participate in the orientation and training of new physicians, APPs, students, and other staff as appropriate
  • Participate in FPPE and OPPE as required
  • Participate in quality improvement projects
  • Become an integral part of the team and provide backup as needed

Qualifications

• Required:
  • Graduate of an accredited APP program
  • Current license as an Advanced Practice Provider issued by the Connecticut Department of Public Health and Addiction Services
  • Active APP certification
  • Excellent general medical clinical skills
  • Ability to provide inpatient and/or outpatient care to adolescent through geriatric patients
  • Clinical assessment and evaluation skills
  • Strong communication and interpersonal skills
  • Knowledge of relevant state and federal laws
  • Adult inpatient medical/surgical or rehabilitation experience
• Preferred:
  • One year of post-training program experience, although this is not required
