Advanced Practice Professional (NP) Pediatric Palliative Care and Partners for Children Community Program
Job Summary
The C.S. Mott Children's Hospital is hiring a part-time Advanced Practice Professional to become part of the community program Partners for Children-Little Heroes League and the hospital-based Stepping Stones Pediatric Palliative Care Consult Team.
This is a combined position, and the APP will be assigned a regular schedule with the Partners for Children and Stepping Stones Consult teams.
Responsibilities
- Hospital and Community program Partners for Children-Little Heroes League:
- Under physician supervision, coordinates care of chronic, complex infants, children and adolescents to children age 25
- Performs patient assessments at intervals of every two (2) weeks to two (2) months via at-home, telehealth, clinic, or in-patient visits
- Creates and updates care plans, conducts and documents yearly comprehensive assessments on patients
- Participates in patient/family/provider/school meetings as an advocate for the patient when needed
- Participates in bi-weekly interdisciplinary team meetings, and weekly rounds to discuss new and active patients
- Communicates assessments and care plans to the primary care provider and specialty consultants
- Orders tests and prescribes medication
- Confers with attending physicians and communicates with other care providers/consultants to coordinate clinical care
- Maintains accurate documentation regarding patient status, evaluations, and treatment
- Performs other related duties as requested by the attending physician
- Utilizes new improvements and/or technologies that relate to job assignment
- Pediatric Palliative Care Inpatient Consult Team:
- Provides comprehensive pediatric palliative care within a collaborative practice model under the supervision of the collaborating physician, in accordance with state regulations and hospital bylaws, across inpatient and ambulatory settings
- Conducts in-person and virtual initial and follow-up consultations for patients from the perinatal period through young adulthood to assess symptoms, clarify patient and family goals and values, and develop care plans
- Performs comprehensive assessments, including physical, cognitive, and psychosocial status
- Manages complex symptoms associated with serious illness using pharmacologic and non-pharmacologic interventions
- Affords goals of care, including advance care planning and hospice eligibility
- Orders, interprets, and recommends diagnostic tests and prescribes treatments based on clinical assessment
- Collaborates effectively with interdisciplinary teams and other healthcare providers across care settings
- Provides education and serves as a resource for patients, families, healthcare professionals, and learners
- Maintains accurate, timely, and compliant clinical documentation
- Participates in quality improvement initiatives and supports the development of departmental education
- Maintains current clinical knowledge and incorporates evidence-based practices and relevant technologies
- Engages in ongoing self-assessment, professional development, and maintenance of licensure and credentials, including participation in professional organizations
- Demonstrates strong communication, organizational, and interpersonal skills
Required Qualifications
- NP: Current licensure to practice as a Registered Nurse and Nurse Practitioner in the State of Michigan, National Certification as a Pediatric Acute Care Nurse Practitioner or Pediatric Primary Care Nurse Practitioner, Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN), Graduation from an accredited Nurse Practitioner program, 3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services, Current DEA registration, AHA Approved BLS
- PA: Current license to practice as a Physician Assistant in the State of Michigan, Satisfactory completion of a physician assistant program accredited by ARC-PA or one of its predecessor agencies, Certification by the National Commission on Certification for Physician Assistants (NCCPA), 3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services, State of Michigan Controlled Substance License Required, Current DEA registration, AHA Approved BLS
Desired Qualifications
- 2-years experience as a practicing Pediatric Advanced Practice Professional
- Experience coordinating care of complex patients with chronic illnesses
- Experience caring for children with high symptom burden and/or complex conditions requiring multiple consultative services
- Advanced Practice Hospice and Palliative Care Nurse Certification and/or Palliative Care training program
- Demonstrated ability to write and communicate clearly
- Demonstrates personal and professional integrity; collaborative and respectful behavior; flexibility
- Excellent communication, teamwork, and interpersonal skills
- Flexibility, willingness to support team and program growth

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
