Advanced Practice Provider (NP or PA): Palliative Care

Responsibilities
- Perform initial assessments and develop Palliative Plans of Care for new members
- Manage a longitudinal panel of 40-50 high-risk patients via telephonic and video encounters
- Provide consultative visits addressing advanced symptom management and goals of care
- Evaluate hospital admissions for trends and develop strategies for readmission prevention
- Participate in regular huddles and meetings with clinical and operations teams
- Support the team during rapid growth and cover roles as needed
- Attend medical meetings to ensure alignment with organizational goals
Qualifications
- Unrestricted advanced practice license in FL, TX, AZ, OH, CO, TN, IL, AL, HI, NC, SC, PA
- 5 years in advanced practice, with at least 3 years in Palliative care
- ACHPN credential or willingness to obtain within the first six months
- Adaptability to time zone flexibility
- Proficient with technology and comfortable in a fast-paced environment
- Strong interpersonal and communication skills
- Experience working within an interprofessional care team
