Summary

The Iowa City Veterans Affairs Health Care System (ICVAHCS) is seeking a Full‑Time Advanced Practice Nurse – Nephrology to join the Specialty Care Service Line at the VAMC in Iowa City, Iowa. This is an open continuous announcement through November 9 – 2026, with the first referral cut‑off on September 7 – 2026 at 11:59 p.m. EST and monthly cut‑offs on the 1st. Applications received after each cut‑off will be reviewed as needed until the position is filled.

Duties

• Coordinate and supervise the delivery of complex care for patients with kidney diseases, focusing on outpatient management of chronic kidney disease (CKD) and hypertension.
• Provide inpatient support for acute consults as needed.
• Conduct outpatient evaluations of patients with CKD and/or hypertension in the clinic and dialysis unit.
• Perform clinic visits and newer models of care, including e‑consults and telehealth visits.
• Obtain history and physical examination, order appropriate labs for diagnostic evaluation, and determine management plans in collaboration with renal physicians.
• Coordinate and participate in implementation of clinical practice guidelines related to CKD care, CKD‑related complications, and hypertension management.
• Provide patient education on management and care of CKD.
• Conduct goals‑of‑care discussions for patients with advanced CKD.
• Oversee referrals to kidney transplantation.
• Tri­age ICVA and inter‑facility consults to the renal service.
• Provide case management support to physicians in the outpatient clinic, dialysis unit, and inpatient renal consultation service as needed.

Work Schedule

• Monday through Friday, 8:00 am – 4:30 pm
• Alternative work hours – if desired – should be agreed upon at the time of hire
