Advanced Practice Provider - Nephrology

Advanced Practice Professional – Full‑time position in York, PA.

Responsibilities

• Join a collaborative team of 6 Nephrologists and 3 Advanced Practice Professionals
• Shared weekday call rotation for dialysis centers
• Manage adult nephrology patients ages 19+ in outpatient, dialysis, and inpatient settings
• Perform dialysis rounding and chronic disease management at three outpatient dialysis centers and WellSpan York Hospital
• Diagnose and treat Chronic Kidney Disease (CKD), Acute Kidney Injury (AKI), ESRD, hypertension, electrolyte disorders, anemia, and post‑transplant kidney conditions
• Develop and manage your own outpatient nephrology patient panel
• Provide in‑basket coverage and collaborate with physicians, APPs, nurses, and dialysis teams
• Travel between DaVita St. Charles Way, DaVita Roosevelt Avenue, U.S. Renal Care Kenneth Road, and York Hospital; mileage reimbursement provided

Qualifications

• Dialysis and nephrology experience preferred; strong new graduate NPs with dialysis RN experience are encouraged to apply
• Acute Care or Critical Care experience is a plus

Schedule

Monday‑Friday, 8:00 AM – 4:30 PM; no evenings, weekends, or holidays.
