About the Role

Essential Functions
- Deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities.
- Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
- Manage chronic conditions and develop individualized treatment plans.
- Serve as the primary care provider and conduct follow-ups.
- Maintain accurate EMR documentation and comply with healthcare regulations.
- Engage patients in disease management and healthy living.
- Collaborate with interdisciplinary teams to ensure coordinated care.
Required Qualifications
- Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients.
- Licensure must be clear of any clinical and/or board practice restrictions.
- Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant.
- Obtain autonomous provider status as required by state you are licensed with within six months of meeting eligibility requirements.
- Two years of experience in a primary care or longitudinal care setting preferred.
- Ability to pass background check upon hire and throughout employment.
- Valid MVR with reliable transportation and clean driving record.
- DEA license and BLS certification.
- Absent other requirements, Arizona market specific: 1+ year of experience in primary care or a related field such as family practice, general practice, or outpatient care.

Competencies
- Self-motivated professional with a strong understanding of risk adjustment models for CMS and ACA.
- Accurately documents assessment findings.
- Maintains patient, team member, and employer confidentiality while complying with HIPAA regulations.
- Demonstrated clinical skills for in-home assessments.
Working Conditions
- High-speed internet service (cable or fiber optic) with minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds.
- Frequent use of computer and phone systems.
- Constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
- Able to lift and carry up to 20 lb.
- Employees may be exposed to varying weather conditions, road hazards, and unfamiliar environments.
