Responsibilities

  • Deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities.
  • Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
  • Manage chronic conditions and develop individualized treatment plans.
  • Maintain accurate EMR documentation and comply with healthcare regulations.
  • Collaborate with interdisciplinary teams to ensure coordinated care.
  • Educate patients on disease management and healthy living.

Requirements

  • Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients.
  • Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner.
  • Obtain autonomous provider status as required by state you are licensed with within six months of meeting eligibility requirements.
  • Two years of experience in a primary care or longitudinal care setting.
  • Ability to pass background check upon hire and throughout employment.

  • A strong understanding of risk adjustment models for CMS and ACA.
  • The ability to document assessment findings accurately and thoroughly.
  • To maintain patient, team member and employer confidentiality while remaining in compliance with all HIPAA regulations.
  • Demonstrated clinical skills necessary to perform in-home assessments.

Working Conditions

  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting).
  • Frequent use of computer and phone systems.
  • Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
  • Ability to lift and carry up to 20 lb.
  • Work is performed in patient homes or facilities and may involve exposure to varied conditions.
  • Employees may be exposed to varying weather conditions, road hazards, and unfamiliar environments.
