Advanced Practice Clinician (NP or PA)

Primary Responsibilities

  • Conduct comprehensive patient assessments
  • Manage complex acute and chronic medical and behavioral health conditions
  • Ensure accurate and complete documentation, including ICD-10 coding
  • Collaborate closely with the interdisciplinary care team
  • Lead advanced illness and advance care planning discussions
  • Provide patient and caregiver education and counseling
  • Participate in facility-based clinical activities and care planning

Required Qualifications

  • Active and unrestricted state NP or PA license, or ability to obtain by start date
  • Master’s degree or higher (NP or PA)
  • National board certification: NP - AANP or ANCC (FNP, ANP, GNP, or AGACNP); PA - NCCPA certification
  • Active DEA licensure/prescriptive authority
  • Ability to lift a 30-lb clinical bag, climb stairs, kneel, and perform hands-on assessments
  • Reliable transportation for daily travel
  • Ability to obtain a collaborative practice agreement if required in the state
  • Full COVID-19 vaccination (per federal, state, and facility requirements)
  • Must pass pre-employment drug screening

Preferred Qualifications

  • 1+ year of clinical experience (long-term care preferred)
  • Experience supporting patients with complex behavioral, social, or functional needs
  • Strong understanding of geriatrics, chronic illness, and end-of-life discussions
  • Proficiency in electronic and written medical documentation
  • Strong communication, teamwork, and time-management skills
