Primary Responsibilities
- Conduct comprehensive patient assessments
- Manage complex acute and chronic medical and behavioral health conditions
- Ensure accurate and complete documentation, including ICD-10 coding
- Collaborate closely with the interdisciplinary care team
- Lead advanced illness and advance care planning discussions
- Provide patient and caregiver education and counseling
- Participate in facility-based clinical activities and care planning
Required Qualifications
- Active and unrestricted state NP or PA license, or ability to obtain by start date
- Master’s degree or higher (NP or PA)
- National board certification: NP - AANP or ANCC (FNP, ANP, GNP, or AGACNP); PA - NCCPA certification
- Active DEA licensure/prescriptive authority
- Ability to lift a 30-lb clinical bag, climb stairs, kneel, and perform hands-on assessments
- Reliable transportation for daily travel
- Ability to obtain a collaborative practice agreement if required in the state
- Full COVID-19 vaccination (per federal, state, and facility requirements)
- Must pass pre-employment drug screening
Preferred Qualifications
- 1+ year of clinical experience (long-term care preferred)
- Experience supporting patients with complex behavioral, social, or functional needs
- Strong understanding of geriatrics, chronic illness, and end-of-life discussions
- Proficiency in electronic and written medical documentation
- Strong communication, teamwork, and time-management skills
