Advanced Practice Clinician (NP or PA)

Primary Responsibilities

  • Conduct comprehensive medical assessments for assigned members
  • Diagnose, treat, and manage acute and chronic medical and behavioral health conditions
  • Provide ongoing evaluation and monitoring of patient status in long-term care settings
  • Perform physical assessments requiring standing, kneeling, and navigating varied living environments
  • Collaborate closely with an interdisciplinary care team including nursing, social work, and facility staff
  • Participate in advanced illness management and advanced care planning discussions
  • Address end-of-life care needs with patients and caregivers in a compassionate and informed manner
  • Ensure continuity of care across multiple facilities
  • Patient and Caregiver Education
    • Counsel patients and caregivers on diagnoses, treatment plans, and preventive care
    • Provide education related to chronic disease management, medications, and health maintenance
    • Support patients with complex behavioral, social, and functional challenges

Required Qualifications

  • Licensure and Certification: Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of residence, or ability to obtain prior to start date
  • National board certification: Nurse Practitioner - Certification through AANP or ANCC in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner; Physician Assistant - Certification through the National Commission on Certification of Physician Assistants
  • Active DEA license with prescriptive authority
  • Ability to obtain a collaborative practice agreement if required by state law
  • Education: Nurse Practitioner - Master of Science in Nursing or Doctor of Nursing Practice from an accredited program; Physician Assistant - Graduation from an accredited Physician Assistant program
  • Minimum education requirement: Master’s degree
  • Physical and Functional Requirements: Ability to lift and transport a 30-pound bag in and out of a vehicle; Ability to navigate stairs and varied facility environments; Ability to sit, stand, and kneel to perform patient assessments
  • Other Requirements: Access to reliable transportation; Full COVID-19 vaccination required in compliance with federal, state, local, and client regulations; Ability to pass a pre-employment drug screening

Preferred Qualifications

  • One or more years of clinical practice experience, preferably in a long-term care or senior living setting
  • Experience caring for patients with complex behavioral, social, and functional needs
  • Strong understanding of geriatrics and chronic disease management
  • Experience with advanced illness and end-of-life discussions
  • Proficiency with electronic medical records and clinical documentation systems
