Position

An exciting opportunity has arisen for an Advanced Nurse Practitioner Primary and Community Care based at East Ayrshire Community Hospital. This post is offered on a permanent basis.

Hours

From 1 April 2026, the working week for NHS Agenda for Change staff in Scotland will be reduced. Full-time hours will reduce from 37 to 36 hours (pro rata for part-time staff) without loss of earnings.

UK Driving Licence

A UK driving licence is essential for this role.

What You Will Do

• Work collaboratively with the multi-disciplinary team
• Support and promote early discharge to a homely setting
• Respond to existing or potential new ill health issues in the person’s home/homely setting
• Contribute to a reduction in unplanned acute hospital admissions
• Work within East Ayrshire Community Hospital to support care delivery by providing a variety of advanced practice care
• Provide advanced levels of autonomous nursing practice to undertake physical assessment, diagnose and plan care or onward referral for people in your care
• Lead and maintain continuity of care
• Work closely with the enhanced integrated care team, ANPs and acute hospital colleagues to support smooth transitions of care to and from home and hospital

Knowledge, Training And/or Experience Required For This Post

• Must be a registered nurse, a qualified Advanced Nurse Practitioner with MSc post grad qualification or equivalent in Advanced Practice or be able to evidence working at Advanced Practice Masters level
• Experience within an appropriate clinical environment
• Demonstrate experience of teaching/education of others
• The successful candidate will be an independent non-medical prescriber
