Advanced Clinical Pharmacist- Transplant
Job Summary
Responsible for developing and leading pharmacy clinical programs in the area of Transplant Services. Provides comprehensive pharmaceutical care to organ transplant donors and recipients by monitoring patient medication regimens and providing appropriate interventions. The pharmacist also collaborates with all medical and surgical specialties responsible for the care of transplant patients, as well as nursing and transplant clinic staff, to ensure optimal care of the patient. Assists in mentoring pharmacy staff, students, and other disciplines as needed.
Qualifications
- Current license from the Texas State Board of Pharmacy or must be licensed by start date if relocating
- Doctorate of Pharmacy from accredited school of Pharmacy required
- Completion of PGY-1 plus 3-5 years in Solid Organ Transplant required or completed/ in the process of completing PGY-2 in Solid Organ Transplant from ASHP accredited
- Specialty BPS certification preferred
- FCCP, FASHP preferred
- ACLS/BLS preferred
