Job Summary and Qualifications

The Advanced Clinical Pharmacist is responsible for providing all levels of pharmacy clinical services. This includes monitoring patient medication regimens and providing appropriate interventions as well as supporting all aspects of pharmacy operations.

In addition to the required duties of a clinical pharmacist, in this advanced role:

• Sustain the hospital formulary by minimizing non-formulary procurements, utilizing therapeutic interchanges, promoting rational drug therapy selection, and performing formulary reviews and presentations
• Attend and actively participate in daily multidisciplinary and bedside rounds
• Work with medical staff to adapt and promote rational drug therapy and make improvements identified by drug utilization evaluations
• Actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership
• Participate and/or lead a clinical program committee at the hospital or division level. Examples include, but are not limited to; antimicrobial stewardship, anticoagulation, critical care, pediatrics, neonates, etc.
• Support and participate in the goals of the organization's quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives
• Participate in the data collection of drug utilization evaluations, adverse drug reactions, and pharmacy quality initiatives and complete notification reports regarding medication errors and adverse drug reactions
• Lead the training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians

Qualifications

• Pharmacist (PHARM-CS)
• Pharmacist - Controlled Substance must be obtained within 90 days of employment start date
• One (1) year experience required
