About the role

The Advanced Practice Provider Float at Prevea Health is an advanced practice nurse (NP) or physician assistant (PA) responsible for delivering health care services to patients, families, and groups. This role emphasizes promoting health and preventing disease under the supervision of physicians.

Responsibilities

  • Provides assessment, evaluation, and management to patients with minor, acute, chronic, and episodic illness and injuries.
  • Collaborates with patients to review their health background and determine physiological, emotional, and environmental needs, advising them on future health plans and illness prevention.
  • Provides case management services to assist clients and families with arranging needed services or referrals.
  • Orders appropriate diagnostic and laboratory tests, prescribes pharmacological agents, and provides other non-pharmacological treatment modalities.
  • Maintains and reviews patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
  • Conducts telephone calls back to patients requiring follow-up care or provides test results. Also provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.
  • Collaborates with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes for patients.
  • Works at multiple sites with variable shifts at the supervisor’s direction to cover leaves of absence, PTO (Paid Time Off), or department demands.

Requirements

This position requires 30 hours per week and floats to Family Medicine, Internal Medicine, Employer Services, and Urgent Care. Primarily located in the Green Bay area but may travel to other regions as needed. Qualifications

  • Graduate of a master’s level nurse practitioner program OR graduate from an accredited physician assistant program.
  • 1-3 years of relevant experience.
