Adult MI Care Coordinator-Conecuh County Jail

Location: Conecuh County Jail, Evergreen, Alabama

About the role

Adult Care Coordinator (case manager) providing case management services to individuals currently in the Conecuh County jail with mental illness diagnosis.

Pay

$35,000+ depending on experience

Qualifications

• Bachelor’s degree
• Valid driver’s license and insurable driving record
• Strong organizational, written, and verbal skills
• Ability to efficiently schedule and carry out appointments

Preferred Experience

• Bachelor’s degree preferably in psychology, social work, criminal justice, or health care management
• Experience working with individuals with mental illness

Responsibilities

• Provide case management services to incarcerated individuals with mental illness
• Conduct needs assessments and complete case plans for consumers
• Work closely with a master-level therapist to determine consumer needs
• Assist consumers with budgeting, housing needs upon release, securing Medicaid/Medicare benefits, medication management, and scheduling/keeping doctor and therapist appointments
• Coordinate with surrounding mental health centers for follow-up services as individuals are released
• Deliver services primarily in jail, home, or community settings, with occasional outpatient visits
• Travel within Conecuh County using an agency vehicle or personal vehicle (mileage paid)
