Adult Medical Home Primary Nurse Coordinator

About the role

KenCrest is seeking a Primary Nurse Coordinator to manage the medical care of the residents within their Adult Community Medical Home (CMH) in Ambler, PA.

Responsibilities

• Participate in the residents’ care teams
• Coordinate and schedule all necessary medical appointments and procedures
• Provide nursing care and supervision to the resident during medical transports
• Complete all necessary care documentation and monthly reporting
• Order and maintain durable medical equipment (DME) supply inventory
• Provide general administrative support to the program as needed

Qualifications

• Registered Nurse (RN/LPN), licensed in the State of PA
• 3 – 5 years' clinical experience, preferred
• 1 – 2 years’ experience with adult tracheostomy and ventilator management, preferred
• Valid Driver's License
• Maintenance of required 30 CEU hrs./2 years
• Current CPR certification
• Clear criminal record and other background checks in accordance with state requirements
