Adult Medical Home Primary Nurse Coordinator

About the role

KenCrest is seeking a Primary Nurse Coordinator to manage the medical care of residents in the Adult Community Medical Home (CMH) in Ambler, PA. The CMH provides a home‑care environment for four medically fragile adults with intellectual disabilities, offering 24/7 full‑circle care to meet social, medical, and emotional needs while promoting the highest quality of life.

Responsibilities

• Participate in residents’ care teams
• Coordinate and schedule all necessary medical appointments and procedures
• Provide nursing care and supervision to residents during medical transports
• Complete all necessary care documentation and monthly reporting
• Order and maintain durable medical equipment (DME) supply inventory
• Provide general administrative support to the program as needed

Qualifications

• Registered Nurse (RN/LPN) licensed in the State of PA
• 3–5 years’ clinical experience (preferred)
• 1–2 years’ experience with adult tracheostomy and ventilator management (preferred)
• Valid driver’s license
• Maintain required 30 CEU hours every 2 years
• Current CPR certification
• Clear criminal record and other background checks per state requirements

Schedule

Monday – Friday, standard business hours with an on‑call rotation.
