Apply by filling out the documents under position details and attach them to the form below Under the general direction of the Adult Medical Rehabilitation and Adaptive Computer & Communication Technology (ACCT) supervisors, responsibilities include monitoring appropriate utilization of program resources, efficient discharge planning, obtaining certifications for rehab stays, ensuring compliance with rules set forth by regulatory agencies, and performing other duties as assigned. The case manager will work as a member of a multidisciplinary team. Education High school diploma or above Experience A minimum of two years working in a role that supported people with disabilities and advocating for their needs.Experience working in a medical rehabilitation facility preferred.Experience working with insurance companies, physician offices, and Medicaid and Medicare preferred. Knowledge/Skills/Licensure/Certification Knowledge of the medical, school and support system people with various disabilities access.Is able to advocate for persons served with public and private agencies, and insurance industry representatives.Excellent interpersonal skills required.CPR certification or eligible.
