Rehab Case Manager

About the role

NHC HealthCare Anderson is seeking a Rehab Case Manager to join their team. The ideal candidate will hold a Bachelor's or Master's degree in social work or equivalent experience.

Responsibilities

• Report to the Director of Social Services on the status of social services and present needs.
• Understand and adhere to the policies of the health care center and interpret them to relevant parties.
• Participate in consultations with the Director of Social Services and the regional social work consultant, addressing problems and trends requiring company policy attention.
• Maintain confidentiality of professional information.
• Exercise independent judgment and follow the Policies and Procedures in the NHC Social Work Services Manual.

Qualifications

• Experience in crisis management is preferred.
• Knowledge of the case management process, needs of geriatric patients, and funding resources such as Medicare, Medicaid, and private insurances.
• Imagination, creativity, and cooperative skills with other professionals.
• Effective communication abilities, both verbal and written.
• Ability to make decisions independently and manage responsibilities efficiently.
• A commitment to the center's mission and goals, and dedication to its success.

Pay

$18.00 to $25.00 per hour, depending on experience and education.
