Adult MH Rehab Case Manager
About the Role
This position provides a variable level of support to consumers with mental health issues, including assistance in accessing medical, social, educational, or other appropriate support services. It involves linkage to more intensive services if needed, monitoring, assessment of service needs, service planning, and coordination. The role carries a caseload of adults and provides Rehab Case Management services within Levels of Care 1, 2, and 3 under the Texas Resilency and Recovery model of treatment.
Responsibilities include ensuring the Recovery Plan is developed based on ANSA calculations and the Authorized Level of Care, frequent in-person contact with individuals, families, and service providers, and intervention in crisis or need situations. The position also involves assessing consumers' living environments and providing spontaneous assistance as needs arise.
This role delivers a basic level of rehabilitative services to improve daily and independent living skills, such as problem-solving, accessing services, securing safe housing, effective communication, socialization, and developing natural support systems. The use of a private vehicle for job duties may be required, and the position must meet monthly performance targets.
Responsibilities
- Provide support to consumers with mental health issues, including linkage to medical, social, educational, or other services.
- Monitor, assess, and coordinate service needs for a caseload of adults.
- Develop and design Recovery Plans based on ANSA calculations and Authorized Level of Care.
- Conduct frequent in-person, long-term contact with individuals, families, and service providers.
- Identify problem or need areas and intervene as required.
- Assess consumers' living environments and provide spontaneous assistance.
- Deliver rehabilitative services to improve daily and independent living skills (e.g., problem-solving, housing, communication, socialization).
- Negotiate and advocate for consumers with external service entities.
- Develop positive relationships with consumers and their Legally Authorized Representatives (LAR) where applicable.
- Utilize community resources to meet individual needs.
- Maintain monthly performance targets.
- Use a private vehicle for job duties and responsibilities as required.
Requirements
- Bachelor's degree from an accredited college or university with a major in social, behavioral, or human services (preference for a degree in Social Work).
- One year of full-time work experience in the human services field, preferably including case management experience (preferred but not required).
- Qualification as a QMHP-CS under the new Medicaid guidelines.
- Work or volunteer experience or expertise in one or more of the following: employment acquisition, housing, budgeting, or community resource acquisition.
- Ability to communicate knowledge of these areas to others.
- Current Texas driver's license and qualification for Center coverage.
- Insured personal vehicle for use in Center duties and responsibilities.
Pay
$18.80 to $19.88 hourly.
Eligible for a one-year step increase, depending on satisfactory performance.
