Adult Intensive QMHP

About the role

Metrocare Services is a mission-driven agency committed to quality gender-responsive, trauma-informed care for individuals experiencing serious mental illness, developmental disabilities, and co-occurring disorders. The QMHP-Adult Intensive position provides psychosocial and rehabilitative services to adults with serious mental illness. The overall goal is to maximize individuals' potential level of functioning, reduce hospitalization, and aid in successful community reintegration.

Responsibilities

• Provide overall service coordination, psychosocial rehabilitation, emergency services/assessment, referral, transportation, linkage, and advocacy to individuals with varying needs.
• Perform duties in concert with other members of an interdisciplinary treatment team.
• Ensure authorization for clinical services.
• Formulate individual recovery plans based on assessment findings for all new admissions.
• Develop measurable objectives and goals agreed upon by the individual.
• Actively assist in obtaining and modifying goals as needed.
• Document all attempts to involve individuals, service providers, and caregivers in the service plan process.
• Review service plans every 180 days or as clinically indicated.
• Participate in interdisciplinary team staffings, presenting psychosocial assessment findings and updating the team on progress.
• Follow up with assigned individuals who miss scheduled clinic appointments and document attempts to contact them.
• Oversee and assist with medication adherence, including med training, arranging transportation, and prompting needed refills.
• Document clinical services by the close of the next business day.
• Perform follow-up hospital assessments the same day as requested and report outcomes to the Clinical Manager.
• Provide crisis intervention/on-call services during the workweek and by weekend rotation, including home and hospital visits, initiating mental illness warrants, and collaborating with police and public servants.
• Facilitate inpatient admissions upon request and ensure continuity of services throughout the inpatient stay, including active participation in discharge planning.
• Evaluate progress of clinical sessions, solicit feedback, and consult with colleagues when dealing with unfamiliar issues.
• Co-facilitate family education workshops.
• Assist individuals in obtaining entitlements through referrals, advocacy, and negotiation.
• Participate in a required on-call rotation.
• Perform other duties as assigned.

Requirements

• Bachelor's Degree from an accredited college/university with a minimum of 30 credit hours in social, behavioral, or human services fields (e.g., psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, special education, gerontology, or educational psychology).
• 1-3 years of mental health-related experience OR a Master's degree in the listed fields with 0 years of experience.
• Current State of Texas Driver License (or ability to obtain within three months of employment).
• Liability insurance if operating a personal vehicle for work purposes; must be insurable by the Center’s liability carrier if operating a Center vehicle.
• Acceptable driving record.
