About the role
Momentum is currently hiring an Adult Day Program Coordinator to join our team. This dual hybrid role involves providing transportation for people supported at Momentum locations in the San Fernando Valley to and from the Day Program in Thousand Oaks.
Responsibilities
- Safely operate the residential transportation vehicles in accordance with Momentum’s Operational Manual
- Safely operate Momentum vehicles on surface streets, freeways, highways, and interstates in compliance with DMV and Highway Patrol protocols
- Operate vehicle lift for people supported in a safe manner and ensure wheelchairs are secured properly. Safety policy requires a 4-point securement for all mobility devices
- Afford assistance with moving in and out of wheelchairs, automobiles, pool, etc.
- Afford assistance with program activities and community-based activities
- Afford assistance with communication, feeding, mobility, toileting, hygiene, and other activities of daily living
- Afford engagement in program activities including computer class, art class, sensory activities, and gardening
- Perform other duties as assigned to meet department and/or business needs
Requirements
- Valid and current proof of legal right to drive in California
- Three (3) years driving experience
- Current DMV driving record printout
- Ability to participate in pool therapy and aquatic activities
- Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
- Ability to run, push, pull, bend, squat, kneel, walk, stand and sit for long periods of time, and lift up to fifty (50) pounds
Qualifications
- A high school diploma, GED, or equivalent
- A minimum of two (2) years of related work experience
- Experience working with individuals with developmental disabilities
Skills and Knowledge
- Must be able to read, write, comprehend, and communicate effectively in English
