ADULT CARE MANAGER

RISE Housing and Support Services is seeking an empathetic, organized, and dedicated individual to join our growing care management team. The role involves working with individuals to develop and progress through goals that help overcome life challenges presented by mental health diagnoses.

Responsibilities

• Developing and implementing care plans with program participants and supporting their efforts to attain their goals
• Complying with all relevant regulations governing the program
• Connecting program participants to community resources and housing options
• Modeling positive and appropriate behaviors and maintaining professional boundaries

Requirements

• Bachelor’s Degree in a human services field
• Two years of experience in a related human services field
• Ability to operate independently, think on your feet, and follow your training
• Excellent organizational and time management skills
• Strong interpersonal boundaries
• Shared belief system in areas such as compassion, respect, positivity, and inclusion
• Successful completion of RISE background checks
• Valid driver’s license and a driving history acceptable to the RISE automobile policy

Benefits

• Starting rate of $22.23 per hour with a 36-hour workweek
• $1,500 bonus after 6 months of employment
