Main Duties & Responsibilities
- Performs history and physical on new/return patients throughout the lifespan.
- Orders appropriate diagnostic/screening tests and reviews results to establish diagnosis; initiates or continues treatment based on findings.
- Manages acute and chronic illness according to established protocols and/or in collaboration with physicians.
- Provides follow-up health care providers or community agencies as necessary.
- Maintains a productivity level necessary to reach mandated annual level of client encounters.
Qualifications
- Education: Master of Medical Science or M.S. in Physician Assistant Studies or an MSN from an accredited program is required; DNP from an accredited program is preferred.
- License: Active PA or APRN license in IL or eligible for IL licensure.
- Board Certification: Active DEA and Controlled Substance Licenses or eligible for DEA and Controlled Substance Licenses.
- CPR (BLS) certification: Active CPR (BLS) certification.
- Experience: Minimum of two (2) years in primary care practice; OR one (1) year of practice as an FQHC primary care provider; OR completion of an APP primary care Fellowship program post-graduation. Experience serving in an FQHC, Community Health Center, or working with traditionally underserved patient populations is highly preferred.
- Language: Ability to utilize interpretation services for patient encounters is required. Fluency in Spanish or a language in addition to English is strongly preferred.
