RWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to southern NJ.

Qualifications

  • CDS and DEABLS
  • Master's degree required or Graduate Level Certification

  • RN License

Essential Functions

  • Performs comprehensive initial and episodic health assessments; in order to identify existing or potential health problems.
  • Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team.
  • Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician.
  • Performs follow-up care including acute and routine examinations.
  • Maintains and adjusts patients medication according to protocol or medical plan of care.
  • Implements nutritional therapies, as appropriate.
  • Triage patient problem calls which requires medical attention.
  • Orders appropriate laboratory and diagnostic procedures.
  • Recognizes normal and abnormal laboratory findings and the pathology involved.
  • Documents information in patients charts following documentation standards.
  • Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching).
  • Manages medical emergencies as indicated, and according to standing orders.
  • Monitors and adjusts patients medication according to protocol or medical plan of care.
