A Bachelor's degree in social work or a related field is required for this position.
Responsibilities
- Ensure a trauma-informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Provide services that result in timely and safe achievement of company and state outcomes relevant to the position.
- Provide direct practice and case management services, including child visits and monthly supervision of the home.
- Help prepare children for adoption and make referrals to the Adoption Therapist.
- Update the child’s social history, child’s plan of service, attend court hearings, complete the sibling split request, and review the child’s file with the selected family.
- Assist in matching adoptive families with children eligible for adoption.
- Support adoptive homes and ensure all required trainings, documentation, and background checks are completed.
- Provide training and preparation for adoptive families.
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
- Back up on-call to the on-call department.
- Provide support for the children served by ensuring they attend their required appointments and by providing transportation if needed.
- Maintain flexibility during business hours to address any unexpected needs of the children served.
