Admissions RN - Hospice

Responsibilities

• Assess potential patients for hospice eligibility and meet with families to evaluate their needs.
• Ensure confidentiality and safety of patient records.
• Complete admissions and patient care orders promptly and accurately.
• Communicate effectively with physicians, referral sources, patients, families, and the interdisciplinary team regarding new admissions and eligibility considerations.
• Coordinate with operational, clinical, and sales leadership on referral status.

Qualifications

• Current unrestricted RN license in the state of operation.
• Minimum of 2 years of experience in hospice, home health, or related healthcare settings.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Preferred Skills

• Current CPR certification or ability to obtain within 90 days.
• Strong fiscal responsibility and computer skills, including proficiency in Microsoft Word and Excel.
