Responsibilities

• Coordinates admissions of hospice patients by performing skilled assessments of patients’ initial physical, psychosocial, and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director, and the interdisciplinary team.
• Provides skilled interventions and instructions to patients and caregivers to ensure optimum symptom control and patient comfort under the direction of a physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and in accordance with policies.
• Submits accurate documentation per care center policy and records pain/symptom management changes and outcomes as appropriate.
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team.
• Communicates with the physician regarding patients’ needs and reports changes in condition; obtains physician orders as required.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license specific to the state(s) of assignment.
• One year of clinical RN experience.
• Current CPR certification.

Pay

$88,000.00–$91,000.00 annual salary range. $7,500.00 sign-on bonus.
