Admissions Hospice RN

About the Role

This is a full-time nursing position supporting hospice patients and their families, with a focus on patient-centered care in the home or skilled nursing facilities. The role involves evaluating patient needs, developing and implementing personalized care plans, and coordinating care with an interprofessional team.

Responsibilities

• Assess patient preferences, barriers, and holistic needs including emotional, spiritual, and cultural factors.
• Develop, document, and communicate individualized care plans aligned with standards and patient goals.
• Promote respectful, equitable, and empathetic care, supporting diverse populations.
• Monitor patient condition changes, update care plans, and collaborate with families and team members.
• Ensure safe, evidence-based nursing practices to achieve quality outcomes.

Requirements

• RN license with a Nursing Diploma or Associate's degree required; Bachelor's degree preferred.
• Less than 2 years of experience; 2-5 years preferred for advanced roles.
• Valid driver’s license; prior hospice or admissions experience is a plus.
• Strong critical thinking, communication, and cultural competency skills.

Additional Information

Travel within the Greater St. Louis area is required, with mileage reimbursement.
