About the role
Treasure Coast Hospice seeks a Hospice Access Coordinator to join our team. This role acts as a liaison between patients/families and healthcare facilities, explains hospice benefits, and educates patients and families about end-of-life care.
Responsibilities
- Explains hospice benefits and appropriateness for the program to patients and families.
- Collects data from patient or designee ensuring clarity and utilization of all eligible hospice services.
- Communicates outcomes of referral meetings to coordinators and submits necessary documentation on the same day of scheduled appointments.
- Maintains professional relationships with healthcare facilities, physicians, case managers, social workers, and nursing staff.
- Works collaboratively with other departments to develop comprehensive program strategies.
- Ensures adherence to legal and compliance issues in hospice care.
Requirements
- High school diploma or equivalent required.
- C.N.A. or M.A. certification or other relevant certification and experience.
- Knowledge of EMR systems.
- Excellent interpersonal communication, presentation, and customer service skills.
- Ability to read, write, and verbally report clinical information to patients, caregivers, and other Hospice staff.
- Must maintain current auto insurance and demonstrate clinical competency upon hire and annually thereafter.
- Must be proficient with computers and complete 12 hours of CEUs annually.
- Minimum two years’ experience in patient care or healthcare marketing, with hospice experience preferred.
- Attendance is a condition of employment.

Schedule
4 days a week, every other weekend from 9am-7pm
