Admission Nurse

Role: Admission Nurse responsible for initiating and resuming high-quality home health services, conducting comprehensive home assessments, and developing individualized care plans in collaboration with the interdisciplinary team. Key Responsibilities Create and update patient-centered home health plans aligning with regulatory and organizational standards.Ensure compliance with all regulatory, payer, and agency requirements.Provide patient and caregiver education, evaluate home safety, and confirm service eligibility.Deliver high-quality clinical care and education with strong clinical judgment and communication skills. Qualifications & Skills Associates Diploma in Nursing required; Baccalaureate Degree preferred.Current RN License in NY (in good standing).At least one year of clinical experience in a Certified Home Health Agency.Knowledge of Microsoft Office and clinical documentation systems preferred.Driver’s License required in all boroughs except Manhattan.Strong assessment, clinical judgment, and critical thinking skills.Ability to work independently in a community setting.
