Admission Nurse

Role Overview: The Admission Nurse is a clinical professional responsible for initiating and coordinating high-quality home health services for patients. They conduct comprehensive home assessments, determine skilled care needs, and develop individualized plans of care in collaboration with the interdisciplinary team. The role involves ensuring accurate documentation, providing patient and caregiver education, evaluating home safety, and confirming service eligibility. The nurse plays a key part in facilitating smooth transitions into home care and promoting positive patient outcomes. Key Responsibilities Create and update patient-centered home health plans of care in collaboration with physicians or authorized practitionersEnsure compliance with regulatory and organizational standardsDeliver high-quality clinical care and education, demonstrating expertise in community health nursing Qualifications & Skills Associate's Diploma in Nursing required; Baccalaureate Degree preferredCurrent RN License in NY in good standingAt least one year of clinical experience in a Certified Home Health AgencyKnowledge of Microsoft Office and clinical documentation systems preferredValid driver’s license (except in Manhattan)Strong assessment, clinical judgment, and critical thinking skillsAbility to work independently in a community setting
