About the role
The Admission Nurse is a clinical professional responsible for initiating and managing high-quality home health services.
Responsibilities
- Conduct comprehensive home assessments
- Develop and update patient-centered care plans in collaboration with the interdisciplinary team
- Ensure compliance with regulatory and organizational standards
- Provide patient and caregiver education
- Evaluate home safety
- Confirm service eligibility
- Create and update home health plans
- Demonstrate community health nursing expertise
- Deliver high-quality clinical care and education
Requirements
- Associate Degree in Nursing (Bachelor’s preferred)
- Current RN license in NY
- At least one year of clinical experience in a Certified Home Health Agency
- Strong assessment, clinical judgment, and critical thinking skills
- Proficiency in documentation and Microsoft Office
- Valid driver’s license (excluding Manhattan)
