Schedule

• Full time
• Day shift (10AM-10PM)

Responsibilities

• Discusses facility core measures during handoff communication
• Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation
• Collaborates with interdisciplinary team members to solve admission and discharge problems
• Collects comprehensive patient medical history
• Establishes and maintains positive relationships with patients and their families
• Provides family members with information about patient condition and plan of care
• Promotes a positive patient experience by demonstrating caring behaviors, maintaining clear communication, and collaborating with the care team to provide safe, timely, and effective whole person care
• Follows hospital policies and procedures, making ethical decisions based on consent and confidentiality
• Collaborates with nursing staff and attending physicians in establishing patient admission and discharge processes
• Ensures compliance with healthcare admission policies and regulations
• Maintains a safe and clean environment for patients
• Manages all aspects of patient admission and discharge processes
• Prepares, completes, and registers patient admissions
• Other duties as assigned

Requirements

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]
• Registered Nurse (RN) license [Required]
• Basic Life Support (BLS) certification [Required]
• Related experience [Required]

Additional Licensure or certification requirements may apply depending on the specific unit or state in which this position is located.

Pay

$33.99 - $63.22 per hour
