Job Description

Responsible for contacting potential patients and physicians regarding services, fielding intake calls, and providing quality customer service to patients. Resolve patient/referral source inquiries, enhance continuity of patient care by providing support to Account Managers and referral sources. Must meet the standards regarding accuracy, timeliness, and professionalism. Consistently communicates with the Account Management and leadership teams regarding new business plans and updates. Provides customer service to patients. Completes pre-surgery calls and/or confirmation calls. Responds to and initiates intake calls to providers, patient care teams and family to gather necessary patient information solely for the intent to process for care. Processes referrals with accuracy. Works with ADONs and the Patient Access Representatives teams to ensure timeliness of processing and compilation of required paperwork for each patient. Provides support to Account Managers in the field. Serves and protects the company by adhering to professional standards, home care policies and procedures. Facilitates the coordination of services to support physicians, patients, caregivers, and/or the community. Utilizes both Summit’s EMR and external EMR systems effectively for accepting, declining and bundling referrals. Meets established KPI’s as assigned.

Qualifications

  • Possess excellent verbal and written communication skills.
  • One year of experience in hospice admissions preferred.
  • Two years of experience in healthcare data.
  • Previous experience in customer service.
  • Experience in home health care, durable medical equipment, or skilled nursing facility preferred.
  • Ability to build rapport with clients and account management.
  • Ability to work independently and as an active member of a team.
  • Strong computer skills in data entry, Microsoft applications, must type 50 WPM.
  • Accurate and precise attention to detail and deadlines.
  • Ability to multitask, prioritize, and manage time efficiently.
  • Goal-oriented, organized team player.
  • High school degree or equivalent.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at a time.
