Acute Care Nurse Practitioner
About the role
We’re seeking a mission-driven Nurse Practitioner to join our team in a part-time role, designed to strengthen care delivery across our centers. The primary focus of this role is to improve same-day access, support high-risk patient stabilization, reduce avoidable acute care utilization, and provide operational support during periods of provider capacity strain. This role is ideal for a clinician who thrives in a collaborative environment and is passionate about improving access, continuity, and the overall patient experience.
The Flex NP is intended to support high-impact clinical access and stabilization needs. Routine longitudinal panel management responsibilities remain with the primary PCP team. Clinicians in this role are expected to exercise a high degree of clinical judgment and autonomy while collaborating closely with market leadership and care teams. You will serve as a trusted clinical partner across care teams, helping ensure that every patient receives timely, high-quality care. You will step into established care teams to support patient needs, maintain continuity of care, and enhance the effectiveness of our primary care providers, without the responsibility of managing your own patient panel.
Primarily assigned to the West Philadelphia hub with periodic travel to regional centers as operational needs arise.
