Acute Care Manager (RN) - Per Diem
The goal
The goal of the Acute Care Manager is to coordinate and manage care to assist patients in meeting their multiple needs, ensure handover and continuity of services, and to decrease fragmentation of care and services. Works collaboratively with members of the interprofessional acute care, ambulatory and post-acute care management teams to improve communication across the continuum of care.

Minimum Qualifications
- A Bachelor's Degree in nursing is preferred with a minimum of 3 years clinical experience.
- Experience: 3-5 years of direct patient care or case management experience in a similar size, similar scope hospital preferred.
Special Training/Skills
- Acting independently, must demonstrate an ability to analyze clinical, financial, and social situations, identifying problems and alternative courses of actions, and directing the healthcare team, with a sense of urgency and a drive
Licensure
- BLS Required;
- ACLS Preferred;
