Essential Duties
- Participate in planning and conducting of individual, small and large group activities.
- Aid in communication between employees, residents, families, support personnel, government agencies, and the public to ensure the best interests of residents, community, and facility are met.
- Engage in community planning related to the interests of the facility and the services and needs of residents and families.
- Develop and maintain a monthly activity calendar and attendance records.
- Collaborate with the Quality Assurance and Assessment Committee to develop and implement corrective actions for identified quality deficiencies.
- Assist with discharge planning, creation, and implementation of activity care plans and resident assessments.
- Arrange transportation for residents, including outings, appointments, and discharges.
- Develop and execute a monthly activity schedule for residents, covering outings, group activities, and in-room activities for bed-bound or isolated residents.
- Encourage residents to engage in self-initiated activities such as hobbies, crafts, and reading, providing necessary materials like Braille or audio books.
- Assist with assessment documentation and quarterly progress notes as assigned by the Activity Director.
- Maintain cleanliness, orderliness, and security within the Activity Department.
Qualifications
- High school diploma or equivalent.
- Preference for one year experience in a long-term care facility.
- Ability to read and comprehend technical procedures and policy manuals.
- Effective communication and presentation skills.
- Basic mathematical skills, including the ability to apply concepts such as fractions, percentages, ratios, and proportions.
- Solution-oriented reasoning and problem-solving abilities.
- Ability to follow instructions and interpret written, oral, and visual information.
