Essential Duties
- Participate in planning and conducting of individual, small and large group activities.
- Aid in communication between employees, residents, families, support personnel, government agencies, and the public to ensure the needs and best interests of residents, the community, and the facility are met.
- Engage in community planning related to the interests of the facility and the services and needs of the residents and families.
- Develop and maintain a monthly activity calendar and attendance records.
- Collaborate with the Quality Assurance and Assessment Committee to develop and implement corrective actions for identified quality deficiencies.
- Assist in discharge planning, creation, and implementation of activity care plans and resident assessments.
- Arrange transportation for residents, including outings, appointments, and discharges.
- Create and manage a monthly activity schedule for residents, including outings, group activities, and in-room activities for bed-bound or isolated residents.
- Encourage residents to engage in self-initiated activities such as hobbies, crafts, and reading, and provide necessary materials like Braille or audio books.
- Aid in assessment documentation and quarterly progress notes as assigned by the Activity Director.
- Maintain cleanliness, orderliness, and security within the Activity Department.
Qualifications
- High school diploma or equivalent.
- Preference for one year experience in a long-term care facility.
- Ability to read and comprehend technical procedures and policy manuals.
- Effective communication and presentation skills.
- Basic mathematical skills.
- Solving practical problems and interpreting instructions.
- Physical demands: standing and walking frequently, sitting occasionally, reaching with hands and arms frequently, talking and hearing very frequently, lifting up to 50 pounds frequently, climbing, balancing, stooping, kneeling, crouching, or crawling occasionally, adjusting focus for vision.
- Work environment: noise level is usually low to moderate.
