Essential Duties
- Participate in planning and conducting of individual, small and large group activities.
- Aid in communication between employees, residents, families, support personnel, government agencies, and the public.
- Engage in community planning related to the interests of the facility and the needs of residents and families.
- Develop and maintain a monthly activity calendar and attendance records.
- Collaborate with the Quality Assurance and Assessment Committee to develop and implement corrective actions for quality issues.
- Assist with discharge planning, activity care plans, and resident assessments.

Qualifications
- High school diploma or equivalent.
- One year experience in a long-term care facility preferred.
Physical Demands
- Standing and/or walking very frequently.
- Sitting occasionally.
- Reaching with hands and arms frequently, pushing/pulling very frequently.
- Talking and/or hearing very frequently.
- Lifting up to 50 pounds frequently.
- Climbing, balancing, stooping, kneeling, crouching, or crawling occasionally.
- Close, distance, color, peripheral, and depth perception in vision.
