Job Responsibilities
- Develop, implement, monitor, and revise Person Centered Plans in collaboration with the interdisciplinary healthcare team.
- Facilitate relationships and serve as a link between the company, parents, guardians, local agencies, and the community.
- Provide supportive counseling to minimize the negative effects of psychiatric symptoms or substance dependence that interfere with the recipient’s daily living and personal development.
- Support clients in developing various skill-building activities, including daily and community living skills, socialization skills, adaptation skills, and behavior and anger management.
- Participate in a first responder on-call system available to consumers and/or their natural support network on a 24/7/365 basis; coordinate “first response" resources according to consumer need and the Person Centered Plan (PCP).
- Perform case management functions of linking and arranging for services and referrals.
- Work closely with other clinical/professional staff to maintain communication, standardize procedures, and expedite PCP implementation.
- Maintain oversight of service utilization to ensure effectiveness, appropriateness, and alignment with service authorizations.
Qualifications
- Education: Bachelor’s degree (not necessarily in Human Services) with 4+ years of full-time experience with the population served, or a Master’s degree or higher with 1+ years of full-time experience with the population served.
- Licenses, Provisional Licenses, Certificates, Registrations, Permits: Must hold a license, provisional license, certificate, registration, or permit issued by the governing board regulating the human service profession, with degrees including Social Service, Sociology, Psychology, or other Human Service fields.

Skills
Must be able to lift a minimum of 15 lbs., pull a minimum of 10 lbs., squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Must be able to stand and walk regularly. Must be able to demonstrate proficiency in CPR from the floor level.
