Education and Experience
- Master’s or bachelor’s degree in a field related or similar to behavioral health
- Bachelor’s degree in a non-related field and one year related experience
- An associate degree in any field and one year related experience
- A high school diploma or equivalency certificate and three years related experience
Licenses and Certifications
- Minimum one year working with seriously mentally ill adults required
- Previous experience working on an ACT team or equivalent experience preferred
- Minimum one (1) year experience working with behavioral health residential programs and/or housing-related services and resources required
Skills
Functions as the housing specialist with primary responsibility for the provision of housing support services in accordance with SAMHSA’s evidence-based practice of permanent supportive housing.
Provides comprehensive assessment of housing, residential and activities of daily living skills and needs, and leads in service planning for adult daily living skills and housing support services.
Collaborates with members in a community setting to coordinate, monitor, and provide direct services.
Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs.
Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.
