ACCS Housing Coordinator - North County Integrated Team C

Description And Responsibilities

The ACCS Housing Coordinator at Adult Community Clinical Services is responsible for:

• Engaging with and developing relationships with individuals to establish treatment goals and address housing barriers.
• Aiding individuals in resolving CORI or legal issues that may hinder housing access.
• Facilitating the transition of individuals into permanent independent housing and supporting them in maintaining this independence.
• Assisting individuals in securing rental subsidies through affordable housing programs.
• Collaborating with the ACCS Integrated Team to set and achieve treatment goals, identify housing and service needs, and remove barriers to housing.
• Supporting individuals through the process of securing and moving into new housing, including setting up and maintaining the household.
• Collaborating with landlords to ensure the apartment is ready for occupancy, conducting move-in/out inspections, and performing ongoing safety and cleanliness inspections.
